A registered voter in Warren County can call the office 309-734-4612 and request a Vote By Mail ballot or they can download the application below, complete and mail to the County Clerk's Office. There is a cut-off date that ballots can be mailed out. We will need the registered voter's name, address, address where ballot will be mailed to and phone number and date of birth.

This is NOT a vote by mail application for one election only. Enrollment in the program remains in effect until you provide notification to cancel or your voter registration becomes inactive.
You must complete and mail the form to the County Clerk's Office. Please select whether to receive a ballot for all elections or only elections that do not require a party designation (General Elections and Consolidated Elections).
